How Do I's


How can my child get counselling?

Bring the following with you to the Adolescent Health Clinic:


    · Picture ID (both client and parent/guardian for first visit)

    · Parent/guardian written consent (clients under 18 years)

    · Accompanied by parent or guardian for first visit (clients under 18 years)


How do I get an antenatal exam?


    1. Visit Adolescent Health Centre on Clinic days. (Tuesdays and Thursdays, 8:30 – 11:00 am.)

    2. Register at front desk.

    3. Wait in the waiting area until name is called.

    4. Participate in examination.

    5. Carry out recommendations prescribed.


Prerequisites:


    · Picture ID (both client and parent/guardian for first visit)

    · Parent/guardian written consent (clients under 18 years)

    · Accompanied by parent or guardian for first visit (clients under 18 years)

    · Infant’s birth registration (postnatal clients)

    · Antenatal passport


How do I get a physical?

    1. Visit Adolescent Health Clinic on clinic days (Tuesdays & Thursdays 8:30am - 11:00am).

    2. Register at front desk.

    3. Wait in the waiting area until name is called.

    4. Participate in examination.

    5. Carry out recommendations prescribed.

How can my baby get immunized?

    1. Visit Centre on Clinic day (Monday 8:00am - 11:00am).

    2. Register at front desk.

    3. Wait in the waiting area until name is called.

    4. Participate in examination.

    5. Carry out recommendations prescribed.


Prerequisites


    · Picture ID (both client and parent/guardian for first visit)

    · Parent/guardian written consent (clients under 18 years)

    · Accompanied by parent or guardian for first visit (clients under 18 years)

    · Infant’s birth registration

    · Infant’s immunization card (if previously immunized)


How do I get a postnatal exam?

    1. Visit Adolescent Health Clinic on clinic days (Tuesdays and Thursdays, 8:30 – 11:00 am)

    2. Register at front desk

    3. Wait in the waiting area until name is called

    4. Participate in examination

    5. Carry out recommendations prescribed

    6. Return for test results

    7. Receive appointment for family planning clinic


Prerequisites:

    • Picture ID (both client and parent/guardian for first visit)
    • Parent/guardian written consent (clients under 18 years)
    • Accompanied by parent or guardian for first visit (clients under 18 years)
    • Infant’s birth registration (postnatal clients)
    • Antenatal passport
