Health Certificate
All applications must be screened at designated sites by the authorized medical personnel. The physician facilitates an initial interview and administers a brief questionnaire. The physician may identify persons who may require more extensive medical examination.
All applicants must pay the prescribed fee and sign the completed questionnaire in acknowledgement of the disclaimer.
Additionally, completion of a Food Safety course has now been included in the requirements for gaining a health certificate. The completed questionnaire administered by the doctor or a receipt of a payment of the prescribed fee serve as a prerequisite for sitting in the course.
Step-by-step Instructions
1. Call the Baillou Hill Road Community Health Clinic to schedule appointment for health certificate and Food Safety Course.
2. Obtain appointment and come 1 hour earlier for registration.
3. Present identification.
4. Fill out necessary form.
5. Wait in waiting area to see the doctor.
6. Participate in "assessment" and interview.
7. Attend Food Safety Course should assessment be favourable.
